What you need to know:

  • There are usually three main root causes for a breach: Malicious or criminal attack, System glitch, Human factor.
  • From Ponemon's 2013 Cost of a Breach report: Engaging consultants actually decreases the cost of a breach.
  • PCI-DSS v3.0 Requirement 12.10: "Implement an incident response plan. Be prepared to respond immediately to a system breach."
  • HIPAA: 164.308 (a)(ii)(6)(ii) Response and reporting - Identitfy and respond to suspected or known security incidents; mitigate, to the extent practicable, harmful effects of security incidents that are known to the covered identity; and document security incidents and their outcomes.
